  In the last 14 days I have not:

Had a cough

Had a fever greater than 100.4°F

Had shortness of breath

Been fatigued

Had muscle aches/pains

Had any new loss of taste or smell

Traveled outside of the US

Traveled outside of CA other than for essential work-related duties

Had any contact with anyone displaying any signs/symptoms of COVID-19

Had any contact with anyone with positive test results for COVID-19
